The Department of Continuing and Community Care (CCC) is a multidisciplinary department of Family Physicians Geriatricians Nurses and Therapist that aims to provide Continuing and Community Care holistic health care to patients in the intermediate and long-term care sector.
As a doctor in the department you will be seeing patients who need their chronic diseases their end-of-life issues their rehabilitation issues or their acute exacerbation’s managed.
We also manage preventative care programme.
The patient may be seen in the community hospital the nursing home in clinics in day centres and in their own homes.
Training will be provided and the doctor can be expected to be guided till they are competent in managing patients in these diverse settings.
The department mainly operates during office hours.
Some staff however will need to be on-call.
This however mainly takes the form of tele-consults.
The doctor must:
be in active clinical practice;
have a basic medical degree from an accredited medical university or medical school;
Possess recognised postgraduate qualifications such as MRCP or equivalent registrable with SMC;
have at least 3 years of post housemanship working experience as a medical officer (or equivalent) in Internal Medicine/Geriatric Medicine or related fields or possess a postgraduate medical qualification in the relevant field/specialty;
fulfill the registration requirements stipulated by Singapore Medical Council (SMC).
