Duties include: - Supporting people leaving hospital to regain their independence in their own homes. - Motivating your customer to work towards the goals they have created - Supporting customers to improve mobility and being part of the community.- Assisting with personal care, (washing and bathing) - Administration of medication - Meal preparation- Light domestic duties Successful applicants will be Drivers that hold a full drivers licence and have their own vehicle.
